Holly Miles headed across the Albion goal from a Helen Dermody free-kick and Alison Hall fired wide of the Baggies’ post.
But it was West Brom who opened the scoring after a dozen minutes, Leigh Dugmore playing the ball over the top for Abi Cottam to smach past Sue Wood in the Coventry goal.
Around the hour mark, Coventry captain Natasha Lynch broke away but shot wide of goal, while Albion’s Paige Stewart saw her effort sail just over the bar.
Amber Hughes then hit the Albion woodwork but in the last minute, Lynch dramatically headed in the equalise.
The final then went straight to penalties and was locked at 3-3, so it went to sudden-death before West Brom were declared the winners of the cup for a second successive season.
